Myelomatous pleural effusion involvement in 23 patients with multiple myeloma: A single-center clinical analysis.
Baseline imaging consisted of a bone survey which demonstrated numerous
myelomatous lesions throughout the axial and appendicular skeleton.
They demonstrated that PTH was capable of increasing bone mass in
myelomatous bones in vivo and that the increased bone formation was associated with reduced tumor burden.
evaluated the value of FDG PET in the assessment of patient with MM and showed that FDG PET has sensitivity in detecting
myelomatous involvement of 85% and specificity of 92%.
With a parallel risk stratification based on
myelomatous molecular changes and in recognition of the importance of these factors collectively in portending risk for myeloma patients, the International Myeloma Working Group (IMWG) published their consensus statement on myeloma risk classification and reported that combination of (a) ISS stage II or III and (b) t(4;14) or deletion of 17p13 would be considered high-risk myeloma on the basis of limited overall survival (OS) of approximately 2 years [24].
Isolated case reports detail the presence of EMP with an uncharacteristically asymptomatic systemic
myelomatous disease [5].
Although typically found within the bone marrow, extraosseous
myelomatous collections referred to as extramedullary plasmacytomas (EMPs) are rarely found in the head and neck region, gastrointestinal tract, and central nervous system.
Ocular manifestations also range from chorioretinopathy (e.g., choroidal infiltrate, choroidal effusion, retinal capillary microaneuryms, hyperviscosity), neuro-ophthalmic abnormalities (e.g., extraocular muscle paresis or direct
myelomatous infiltrate, papilledema), and corneal deposition (crystal or copper) to opportunistic infections of ocular structures.
These results were consolidated by studies using bone marrow specimens from myeloma patients in whom HLA-G limits CD138- stem cell differentiation into [CD138.sup.+]
myelomatous tumor cells.
