KMS was considered in the patient who had wide spread vascularized giant hemangiomateous mass with thrombocytopenia and consumption coagulopathy. Embolisation operation was made on inner-mass external karotis branches by Interventional Radiology unit and external karotis and its branches feeding the mass were embolized with a ratio of 80% with the operation.
Conclusion: We wanted to share a rare KMS case occurring together with thrombocytopenia and consumption coagulopathy.
Although post-transfusion laboratory findings were not reported and consumption coagulopathy did not seem to be aggravated in the authors' patient, I would like to draw attention to the complication of substrate supplementation in consumption coagulopathy cases without the taking of necessary precautions.
(2) The diagnosis is based upon three basic findings; enlarging hemangioma, thrombocytopenia and consumption coagulopathy. The thrombocytopenia and consumption coagulopathy is known as Kasabach-Merritt phenomenon.
This triad of hemangioma, thrombocytopenia and consumption coagulopathy led to the diagnosis of KMS.
Thawed plasma has lower levels of factors V and VIII and is not indicated in patients with consumption coagulopathy (Diffuse intravascular coagulation).
Hypercoagulation can be due to procoagulants in the venom, such as arginine, esterase and hydrolase, and consumption coagulopathy phase--DIC.
