Like: Double Crosshairs; No Particles for Weapons; No Effects of Explosions.

Exact same thing is happening for me too. No performance issues or anything, just, weird bugs.

Edit: A quick workaround for the double crosshairs, open console and type "hud_quickinfo 0" However the gravity guns crosshair is still a big weird rectangle.

Game settings don't save...
Mmod setting too...
Like: regen HP or Turn On/Off Better Combine Aim...
They are reset all the time when i rejoin the game...
Any fixes?

There is a config.cfg file in the patch's VPK that overwrites the user's settings upon loading a map or restarting the game. Deleting it solved the problem for me.
