DS 1500

DS 1500

A form used by GPs in the UK which allows for immediate release of certain funds (Disability Living Allowance, Attendance Allowance or Incapacity Benefit under the so-called “special rules”) for a patient with a terminal illness. The DS 1500 is issued on request by a patient (or his/her representative), and contains details about the patient’s diagnosis, current and proposed treatment, and details on clinical findings.
